Student Club Activation Form

ASFRC is the governing body for Student Clubs. All clubs must be approved through the ASFRC before earning status as an official club. Each year the Advisor for the established club must complete an activation form. This will be used to maintain updated information on the website and to share with students.

Club Advisors are responsible for hiring student reps and submitting the Club Summary at the end of the semester to receive the Advisor Stipend. Your signature in the box below demonstrates that you understand and agree to these expectations.
