Laptop Request Application

Each semester the Basic Needs program will purchase a limited number of laptops for students to request. This form will only be available while supplies last. To qualify for a personal laptop students must meet the following criteria:

-Have a completed FAFSA AND at least $250 of unmet financial need
-The student must have a declared major and be seeking a degree
-Have a Student Education Plan (SEP) on file
-Have a cumulative GPA of 2.0 or higher
-Provide an educational goals statement as a part of this application

The statement should include specific examples used for academic tasks, such as: research projects, coursework assignments, online learning, or other relevant academic activities
I understand that I must meet the above listed criteria to be eligible for a laptop and that if I am to be awarded a laptop that it should exclusively be used for academic purposes. I take full responsibility of the laptop and acknowledge I am responsible for any required repairs or any software that may be needed. I acknowledge that if the laptop is damaged or stolen it is my responsibility to fix or replace.
